What submitting claims for a therapist under supervision looks like:

  2. You create an account (including entering your license type).

  3. You link your supervisor automatically under Supervisor Details:

If you have your own solo practice.

  1. You create an account (including entering your license type).

  2. You link your supervisor manually.

  3. You submit a claim like you normally would!

Here’s what it looks like on the claim (CMS1500) form

There are three boxes where an NPI is identified:

  • Box 33: Group practice NPI (if solo practice, individual NPI will be listed here)

  • Box 24: Rendering provider NPI

  • Box 17: Supervisor NPI
